Please include the following information with your shipped equipment.

- Your full name, mailing address and daytime phone number.
- A packing list (typewritten or printed preferred) including all contents of the shipped equipment (Power cords, footswitches, other accessories, etc.)
- A Bill of Sale for any warranty repairs.
- A description of the equipment failure (e.g.- Does not power up, crackling and distorted sound from channel 2, etc.)
- Any special instructions (e.g.- written request for estimate, repair as soon as possible, etc.)

FedEx, UPS and Airborne Express are reputable carriers with drop-off locations or pick-up service available throughout the US. Visit their web sites to run down a checklist for delivery, find a drop-off location or schedule a pick-up at any US location.
